Linear footage first. A 1,400 square foot ranch might have 120 feet of gutter; a 3,000 square foot two-story can have 250 feet or more, and that is most of the price difference right there. Then height, because a second story means more ladder setups, more repositioning and more time per foot. Then condition: a gutter that has not been touched in three years takes materially longer than one on an annual schedule, and some companies charge a neglect surcharge for it.
A proper gutter clean is not just the trough. It should include every downspout flushed and tested so water runs clear, debris bagged and removed rather than blown onto your roof and lawn, and a look at the hangers, seams and fascia while the crew is up there. If a quote does not mention downspouts, ask, because a clear trough feeding a plugged downspout is not a working gutter system.
Blowing gutters out with a leaf blower is faster and cheaper for the company and worse for you. It scatters wet debris across the roof, the yard and the windows, and it does not clear a packed downspout at all. It is cheap insurance against an expensive problem. A blocked gutter overflows against the fascia and the foundation, and in an Idaho winter a trough full of frozen debris pulls hardware off the house and starts ice dams that push water back under the shingles. A $200 clean twice a year is a rounding error next to fascia replacement or water in a basement.

No. Guards reduce how much reaches the trough and they help most with large leaves. Fine debris, shingle grit and cottonwood fluff still get through or mat on top of the mesh, and guarded systems still need periodic clearing. They lengthen the interval, they do not remove the job.
